CICCC Update—18 May 2021 Meeting

Review of Incidents at all Australian facilities of Quantem for 2020 and 2021 (February to April)
Ryan de Munk, General Manager HSEQ, provided a significant review of incidents over all of Quantem’s Australian facilities for the years 2020 and 2021 to date. Of particular interest was the use of “spider charts”, which gave a graphical display of the incidence of the various categories of incidents over each year. Specific spider charts for the West Melbourne facility alone were also presented. These showed that near misses and asset integrity were the two areas with the greatest incidence. Ryan stated that the number of near misses was well up, and this was a most desirable trend as it shows that potential safety matters were being well reported. He also advised that overall both the number of incidents and the number of management actions had increased, while the severity of incidents was declining. In 2020 there were 122 incidents and 127 actions, a ratio of 1.1 actions per incident. In 2021 (Jan to April) there were 49 incidents and 83 actions, a ratio of 1.7

Retirement of CICCC Chair
Robin Saunders, who has chaired the CICCC since its inaugural meeting on 21 May 1998 retired after the May 2021 meeting. The Community Members warmly congratulated Robin on his work over the past 23 years, and wished him well in his retirement.

The next meeting will be held on Tuesday 10 August 2021 at 3pm, at a venue to be decided
