Incidents on site

One particular incident at the facility was discussed at some length. It involved a weep of potassium hydroxide from tank number 46, which was noticed on the ground right next to the tank base. The product was pumped to another tank, and after the empty tank was cleaned, the floor plate welds were examined and found to have some small fatigue cracks. A review of previous ten year inspections did not show any problem with the welds.

Community representative Faye Simpson suggested that a data base be commenced to look for trends should such events as this occur again. Carlo Fasolino, the Terminals’ Victorian Manager, advised that welding techniques have improved over time, making it difficult to get much out of a comparison of historic records. However Carlo suggested another approach, and advised that he would get an independent third party to review the recent inspection report, and the one 10 years ago, to see if there are any anomalies between the reports on welds on the bottom plates of Tank 46.

Terminals Maintenance System

Community representative Ian Thomas invited Terminals Pty Ltd to make a presentation on its maintenance management system to the Asset Management Council. Carlo requested that further details be provided, at which time the invitation would be considered.
