CICCC Update—22 May 2018 Meeting

New appointments at Terminals Pty Ltd

Ryan de Munk has been appointed to manage the Health, Safety and Environment areas at Terminals Pty Ltd. Ryan attended the meeting, and advised the Committee about his background and qualifications. He has a degree in Health and Safety, an MBA and 20 years experience in Health & Safety. He worked previously for Wesfarmers in the Chemical, Energy and Fertiliser business, with LPG & LNG that was extracted to the Dampier to Bunbury gas pipeline. Stephen Lewis has moved across to Safety Standards and Cases Manager.

Recent Pin Notices from WorkSafe

Ryan reported on the eight Improvement Notices (IN) served on the Company within the last 12 months. These arose principally from WorkSafe inspections on site. For example, when a WorkSafe inspector queried the position of a valve at the truck loading bay during a check on the fire fighting foam. It transpired that a complete shut down of the foam delivery system occurred when the checking was being done, leaving other areas such as the tank loading station unprotected. The various actions required by the IN’s have been completed and WorkSafe is satisfied.

Incident reporting in the three-monthly report to the CICCC

Phil Jones took the Committee through the new reporting using the new ‘Integrum’ system which describes how incidents are managed. The information provided is very extensive and is tailored to the CICCC’s needs.

One incident was detailed involving loss of containment of a storage tank containing Aluminium Sulphate. The incident involved a spill of product which was captured and retained by the facility’s bund compound.

The Aluminium Sulphate was delivered in an unexpected condition which caused the material to become corrosive resulting in the tank leaking into the protective compound bund. There was some subsequent seepage into local groundwater and a clean-up is being carried out with ongoing monitoring to the satisfaction of the EPA.

The EPA issued an infringement notice (relating to the leak) and internal inquires and remedial actions have been proactively taken to ensure there is not a repeat.
